We’re seeking an experienced Level 1 RN Case Manager to join a healthcare team in Oklahoma. This role is based in an ambulatory health care setting, working as an essential member of a collaborative care team that may include providers, licensed practical nurses, medical assistants, and patient panels.

Start Date: ASAP

Schedule: Full-time, Day Shift (hours may vary by facility)

Setting: Ambulatory Health Care

Requirements

Active Oklahoma RN license or Compact/Multi-State RN license

Minimum 3 years of RN experience

Certifications:

CPR, BLS, ACLS

Strong written and verbal communication skills

Ability to meet pre-employment credentialing clearances (Background Check, Drug Screen, etc.)

Responsibilities

Conduct pre-visit planning for scheduled patients

Communicate patient needs and recommended treatment plans to care team staff

Maintain & update the team’s patient panel

Identify, create, and manage high-risk subpopulation panels for care management

Track and report monthly performance measures to staff and patients

Participate in quality improvement initiatives using the Model for Improvement

Contact patients for follow-up diagnostic tests, referrals, appointments, or medical care

Schedule nursing visits and follow up on referrals

Provide patient education and support self-management strategies

Triage walk-in or call-in patients and direct appropriately

Document all care activities in compliance with documentation standards

Provide direct nursing care backup as needed by the team
