Qualified Developmental Disability Professional (Talladega)
AltaPointe Health Systems is seeking a Qualified Developmental Disability Professional to support clients in the Talladega area.
Essential Functions- Coordinate Individual Support Team (IST) functions and responsibilities
- Plan and schedule meetings based on the preferences of the person receiving services regarding time, place, and participants
- Assist people to lead their meetings consistent with their preferences and abilities
- Actively engage people to increase participation in all aspects of their programming
- Develop individual program plans using Personal Outcome Measures (POM), interviews, team member feedback, and required assessments to determine the person’s needs, goals, and desires
- Review the effectiveness of individual program plans and services, making timely modifications as needed
- Serve as the designated contact for people within the assigned caseload, their parents, guardians, and authorized representatives
- Maintain documentation of people’s needs for supervision and support
- Ensure people have annual physical exams and that recommendations are implemented for optimal health
- Keep face sheets and contact information accurate and up to date
- Ensure psychotropic medications have monitoring plans and that plans are reviewed periodically per DMH standards
- Ensure behavior plans, if applicable, are implemented and monitored per plan and DMH standards
- Train program staff in the implementation of individuals’ program plans
- Identify and secure community opportunities that support people’s interests
- Ensure safety and community acceptance of people
- Oversee the development of weekly individual and group schedules with direct input from participants
- Complete after‑event evaluations of activities to gauge feelings and improve future planning
- Provide individual schedules to people, families, guardians, providers, and support coordinators as requested
- Notify the Program Manager immediately when an activity venue cannot be accessed
- Maintain familiarity with emergency procedures and keep each person's emergency information up to date
- Report, monitor, and document accidents, injuries, behavior incidents, and health concerns
- Maintain updated program files with all required documents for the caseload
- Complete all requirements within specified timelines
- Respect confidentiality of personal information per AltaPointe policy and procedures
- Seek supervision and consultation as needed
- Accept and employ suggestions for improvement
- Actively work to enhance skills
- Attend in-service training and professional development activities as required by DMH, AltaPointe, or the Division Coordinator
- Treat individuals with care, dignity, and compassion
- Respect individuals’ privacy and confidentiality
- Be pleasant and cooperative with others
- Assist individuals and visitors as needed
- Personal values should not inhibit the ability to relate and care for others
- Be sensitive to individuals’ needs, expectations, and differences
- Be gentle and calm with individuals and families
- Participate actively in quality/performance improvement activities
- Participate actively in AltaPointe committees as required
- Complete assigned tasks in a timely manner
- Follow AltaPointe policies and procedures
- Any other duties as assigned by supervisor or designee
Bachelor’s Degree (BA/BS) in a human services field (Psychology, Sociology, Special Education, Social Work, or a related field), or a Registered Nurse with a minimum of one year’s experience working with individuals with developmental disabilities is required. Candidates must meet Department of Mental Health guidelines for designation as a Qualified Developmental Disabilities Professional (QDDP) and all driving requirements. Within six months of employment, applicants must effectively demonstrate the application of population‑specific competencies.
- Ability to interpret and implement support plans and motivate, instruct, and supervise persons with intellectual disabilities.
- Knowledge of psychiatric disorders and intellectual disabilities in the adult population.
- Knowledge of cultural diversity.
- Knowledge of available resources.
- Knowledge of behavior management, crisis intervention, and non‑violent crisis intervention practices for individuals with intellectual disabilities.
- Knowledge of applicable standards (DMH, Joint Commission, Medicaid).
- Knowledge of Person‑Center Planning, natural supports, and the philosophy of self‑determination.
- Knowledge of utilizing electronic incident reporting.
