Registered Nurse - Case Manager

REGISTERED NURSE - CASE MANAGER – Progeny Human Resource Solutions.

Location:

Tallahassee, FL. Salary: $70,500 – $96,900.

• Facilitate interdisciplinary plan of care focusing on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management.
• Coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
• Identify potential barriers to patient throughput and quality outcomes and facilitate appropriate discharge plans.

• Minimum of 3 years of recent case manager experience in an acute care setting. Experience can be on Med/Surg, Tele, Neuro, ICU, PCU, or ED units. Candidates with home health or insurance case manager experience and 3 years acute care total experience (at least 1 year within last 5 years) will also be considered.
• Associate’s Degree in Nursing or Nursing Diploma required;
  Bachelor’s Degree preferred.
• Current Florida RN license required or appropriate compact licensure; compact license must be valid within 90 days of hire.
• Advanced Practice Registered Nurse license acceptable if current and compliant.
• Certification in Case Management, Nursing, or Utilization Review preferred.
