Responsible for assignment and coordination of staff, day‑to‑day workflow, and departmental operations to ensure appropriate and cost‑effective patient care.
Responsibilities- Ensure all admissions and continued stays meet clinical criteria for appropriateness and medical necessity.
- Assess discharge planning needs and coordinate delivery of care to meet these needs.
- Develop and coordinate individual patient case‑management plans to ensure appropriate level of care across the continuum.
- Collect and analyze patient and family data, expedite and coordinate services to facilitate patient progression through the healthcare system.
- Initiate the case‑management process to achieve timely, cost‑effective, medically necessary treatment and discharge planning.
- Perform admission certification screening and concurrent stay reviews using ISD criteria; maintain documentation on discharge planning notes.
- Evaluate patient educational needs regarding continuum of care; provide information on community resources, discharge options, and pre‑/post‑treatment options and costs to patients and families.
- Registered Nurse, or Advanced Practice Registered Nurse (APRN).
- Registered Nurse Diploma or Associate Degree.
