Responsibilities:- Provide direct patient care to a diverse population of residents with varying needs and conditions
- Assess and develop individualized care plans in collaboration with the interdisciplinary care team
- Administer medications and treatments as prescribed by physicians
- Monitor and report changes in residents' conditions to ensure prompt intervention and treatment
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Participate in performance improvement initiatives to enhance the quality and efficiency of care delivery
