This role allows you to work one-on-one with patients, restore their independence, and be part of a supportive and passionate care team.

What You’ll Do

• Deliver in-home physical therapy as prescribed by physicians
• Evaluate and treat patients with functional impairments
• Promote independence and improved quality of life
• Develop customized care plans and document patient progress
• Supervise Physical Therapist Assistants and ensure care quality
• Collaborate with the interdisciplinary team to optimize outcomes

What You Bring

• Graduate of an accredited Physical Therapy program
• Current Physical Therapy license in Pennsylvania
• Passion for compassionate, patient-centered care
• Strong communication and problem-solving skills
• Ability to work independently and as part of a care team
