Physical Therapist (PT) - Home Health Full-Time | Flexible Schedule
About the Role We are seeking a skilled and compassionate Physical Therapist (PT) to join our home health team. In this role, you will provide one-on-one therapy services in the comfort of patients’ homes, helping them recover, regain independence, and improve their quality of life. You’ll work with a diverse patient population, tailoring treatment plans to meet individual goals and functional needs.
Key Responsibilities

• Conduct evaluations to assess patient needs and create individualized treatment plans.
• Provide therapeutic interventions focused on mobility, strength, balance, and functional independence.
• Educate patients and families on safe practices, exercises, and home modifications.
• Collaborate with an interdisciplinary care team to ensure comprehensive patient care.
• Accurately document patient progress and update care plans as needed.

• Current state licensure as a Physical Therapist (PT).
• Strong clinical and assessment skills.
• Ability to work independently and manage a flexible schedule.
• Compassionate, patient-centered approach to care.
• Prior home health experience preferred, but not required.
