Community Dietetic Assistant Practitioner - 28.5 hours per week - fixed term until October 2026
As part of the Community Dietetic Service, you will assist the registered dietitians with the assessment, implementation and monitoring of nutritional care plans for adult patients referred to the service.
You will spend time with patients and carers to optimise patients nutritional intake via implementing and monitoring nutritional care plans.
The role is community based, and you will see patients in the intermediate care unit (ward based) nursing homes and residential homes. This post is a fixed term maternity cover.
Main duties of the jobTo independently undertake nutritional screening/assessment of appropriate patients once triaged by the Dietitians.
To support the dietitians by reviewing and monitoring oral nutrition support patients in a range of community settings, independently amending care plans.
To develop nutrition care plans for appropriate patient and be responsible for carrying out reviews in a timely manner.
To work independently whilst being aware of own competencies.
To independently prioritise patients within your caseload.
To have a working knowledge of nutritional supplements and request prescriptions appropriately.
To be able to interpret food / fluid record charts and weight charts and make appropriate suggestions to patients / nursing staff if changes are required to the care plan.
To calculate body mass index (BMI) and interpret the result taking appropriate action.
To order nutritional supplements for the intermediate care unit, ensuring sufficient stock to meet patients' needs.
To have good knowledge of nutrition including macro nutrients.
To be able to analyse food charts, calculating current intake and using judgement adapting plans to meet patients' needs.
To have good verbal and written communication.
To be aware of and maintain confidentiality in the workplace under the data protection act.
