Job Title Advanced Clinical Practitioner (ACP) / Advanced Nurse Practitioner (ANP).
Location: Ashton Medical Group, Ashton-under-Lyne.
Reports To: Clinical Leadership Team / GP Partners.

The Advanced Clinical Practitioner (ACP) / Advanced Nurse Practitioner (ANP) will work within the Ashton Medical Group’s same‑day triage hub, providing assessment, diagnosis, treatment and management of patients presenting with acute and urgent primary care needs.

The post holder will work autonomously within their professional scope of practice and as part of a wider multidisciplinary team to ensure patients are directed to the most appropriate clinician or service at the right time.

The role includes telephone triage, online consultation assessment and face‑to‑face clinical assessment where appropriate.

The successful candidate will support safe, effective and efficient patient care within a fast‑paced primary care environment.

Main Duties and Responsibilities Clinical Responsibilities
  • Assess, diagnose and manage acute and urgent presentations within primary care
  • Undertake telephone, digital and face‑to‑face consultations
  • Provide autonomous clinical decision‑making within scope of practice
  • Prescribe medication independently where qualified
  • Request and interpret investigations as appropriate
  • Develop and implement management plans for patients
  • Identify red flags and appropriately escalated complex or deteriorating patients
  • Provide appropriate safety netting and follow‑up advice
  • Maintain accurate and contemporaneous clinical records
  • Work in line with local and national clinical guidelines and protocols
Triage Hub Responsibilities
  • Support delivery of the same‑day triage model
  • Assess incoming patient requests and determine appropriate clinical pathways
  • Prioritise patients based on clinical need and urgency
  • Work collaboratively with GPs and the multidisciplinary team to ensure effective patient flow
  • Contribute to operational efficiency within the triage hub environment
Team Working
  • Work collaboratively with GPs, nurses, pharmacists, mental health practitioners, paediatric clinicians, MSK practitioners, care coordinators and administrative teams
  • Support and mentor colleagues where appropriate
  • Participate in multidisciplinary meetings and clinical discussions
  • Promote a positive and supportive team culture
Quality Improvement and Service Development
  • Participate in audit, quality improvement and service development work
  • Contribute ideas to improve patient care and operational effectiveness
  • Support implementation of new pathways and innovations within primary care
Professional Responsibilities
  • Maintain professional registration and revalidation requirements
  • Participate in mandatory training and continuing professional development
  • Work within professional codes of conduct and organisational policies
  • Maintain confidentiality and information governance standards
  • Act in accordance with safeguarding policies and procedures
Person Specification

Essential Criteria
  • Registered healthcare professional with relevant professional body registration
  • Advanced Clinical Practice qualification or equivalent experience
  • Experience managing acute presentations within primary care, urgent care or equivalent settings
  • Excellent clinical assessment and decision‑making skills
  • Ability to work autonomously within scope of practice
  • Strong communication and interpersonal skills
  • Ability to work effectively within a multidisciplinary team
  • Good organisational and prioritisation skills
  • Commitment to patient‑centred care and continuous improvement
Desirable Criteria
  • Independent prescribing qualification
  • Experience working within primary care triage models
  • Experience using digital consultation systems
  • Interest in service development, teaching or leadership
  • Experience participating in quality improvement initiatives
Working Pattern

Sessions and working pattern are negotiable depending on experience and service requirements.
