We are seeking a motivated and compassionate Practice Nurse to join our friendly and supportive team at Staveleigh Medical Centre. In this role, you will undertake a wide range of practice nursing duties, including long-term condition management, immunisations, and cervical screening. Main duties of the job

The successful candidate will be responsible for delivering high-quality, patient-focused nursing care within a primary care setting. This includes supporting patients with the ongoing management of chronic conditions, promoting health and wellbeing through preventative care, and carrying out routine clinical procedures.

You will work collaboratively with the wider multidisciplinary team to provide safe and effective care, maintain accurate clinical records, and contribute to the continuous improvement of services. The role also involves engaging with patients to provide education, advice, and support to help them make informed decisions about their health.

Job responsibilities

Job Description Practice Nurse

The Practice Nurse will work as part of the primary healthcare team at Staveleigh Medical Centre, delivering high-quality, evidence-based nursing care to the practice population.

Key Responsibilities:

  • Deliver general practice nursing services and other clinical procedures
  • Support the management of long-term conditions such as diabetes, asthma, COPD, and hypertension, including patient reviews and care planning
  • Administer immunisations and vaccinations in line with national and local guidelines
  • Undertake cervical screening in accordance with current protocols
  • Promote health and wellbeing through opportunistic advice and structured health promotion clinics
  • Provide patient education to support self-management of conditions
  • Maintain accurate, contemporaneous clinical records in line with practice policies and professional standards
  • Work collaboratively with GPs, healthcare assistants, and the wider multidisciplinary team
  • Participate in infection prevention and control procedures
  • Support quality improvement initiatives and contribute to audit activity where appropriate

  • Maintain up to date knowledge and competencies relevant to the role
  • Work within the NMC Code of Conduct and scope of professional practice
  • Participate in ongoing training, appraisal, and professional development
