Description:

We are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.

Key Responsibilities:

Manage a caseload of up to 35 patients with severe and enduring mental health conditions Act as Care Coordinator in accordance with the Care Programme Approach (CPA) Complete comprehensive mental health assessments and risk assessments Develop, implement, and review person-centred care plans Work collaboratively with the multidisciplinary team and external agencies Provide therapeutic interventions and ongoing support to service users Maintain accurate clinical records and documentation in line with NHS policies Promote recovery, independence, and positive mental health outcomes Requirements:
Valid NMC Registration (Registered Mental Health Nurse) Previous Community Psychiatric Nursing (CPN) experience Experience managing a community mental health caseload Knowledge of CPA, risk assessment, and safeguarding procedures Up-to-date mandatory training Enhanced DBS Available to start ASAP Referral Bonus If you know a nurse who would be interested, please refer them — you’ll receive a referral bonus once they complete the required hours.
