Provide case management for clients in the homeless program who may be diagnosed with chemical dependency or co-occurring chemical dependency and mental health disorders by performing the following duties.

• Administer a standardized housing intake.
• Develop a housing plan with each client and coordinate with appropriate programs depending on housing placement.
• Develop transition and discharge plans with clients that include provisions for housing, employment and behavioral health disorders.
• Provide 1:1 CM activities to assist clients in meeting service goals by meeting weekly and/or biweekly.
• Facilitate life skills group with fidelity to evidence-based practices.
• Prepare and maintain clinical records within required time frame.
• Maintain meaningful involvement with consumers, advocates, and community partners.
• Submit reports as required.
• Participate in client staffing and team meetings.
• Make referrals to appropriate resource agencies and document outcomes of referrals.
• Knowledge of program policies and procedures.
• Provide follow-up and aftercare services as needed.
• Follow all established Agency policies and procedures.

• Maintain client rights as defined by DCF.
• Maintain the highest standards in ethical conduct and confidentiality.
• Maintain clinical records in accordance with Agency standards.
• Follow all established Agency policies and procedures.

Undergraduate Degree in a related field.

Two Years.

• Ability to read and interpret documents such as safety rules, operating instructions, and procedure manuals.
• Ability to write routine reports.
• Ability to speak effectively.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Knowledge of homeless services preferred, 12-step program, behavior modification, case management and addiction.
• Knowledge of Rental Assistance programs, Rapid Rehousing Funding, Permanent Supportive Housing.
