Overview

This is a professional social services position for the Medical Foster Care (MFC) program reporting to the Registered Nursing Supervisor. The individual in this position utilizes the social work process to provide pediatric and family social work expertise in the assessment, training, implementation, and evaluation of MFC program services. This position is designated as on-site, though it may begin remotely until the on-site transition is implemented.

Responsibilities
  • Complete pre-service assessment, interview, and written assessment of prospective MFC parents and their home environment, including home visits.
  • Complete annual and as-needed written assessments of MFC parent providers services from the social work and administrative perspective.
  • Conduct quarterly MFC parent and child/youth dyad assessments to evaluate strengths, skills, and needs for quality and safety.
  • Provide at least quarterly comprehensive review of MFC parent provider documentation requirements.
  • Perform home visits and community visits for assessment purposes.
  • Assess availability of MFC beds statewide, conduct bed searches, and track responses to requests.
  • Complete psychosocial summary information and follow up as indicated by local area offices and central office.
  • For fee‑for‑service Medicaid clients, conduct MFC child/youth social work assessments.
Communication
  • Provide verbal and written communication with MFC and community stakeholders to ensure understanding and engagement.
  • Facilitate coordination in the scheduling of MFC staffing and engage necessary stakeholders.
  • Engage, advocate, and support caregiver participation and family‑centered approaches.
  • Provide psychosocial and administrative education and documentation of training and technical assistance.
  • Respond to regional and statewide requests for information about MFC program coordination.
  • Act as the MFC program liaison with other community providers and stakeholders.
  • Serve as liaison between the MFC provider and the MMA Plan.
  • Disseminate assessments and reports, as appropriate, to MFC program stakeholders.
  • Attend and participate in other meetings as assigned.
Education & Training
  • Provide pre‑service MFC training for foster parents and program onboarding training.
  • Coordinate and oversee annual psychosocial and administrative training requirements.
  • Track program training requirements to ensure completion.
  • Provide community outreach and education to stakeholders regarding the MFC program and requirements.
Implementation
  • Coordinate all social services and administrative day‑to‑day operations of the MFC program.
  • Review MFC program referrals and documentation to determine availability of an appropriate MFC parent provider.
  • Prepare and coordinate the admission of MFC child/youth into the program and parent home.
  • Assist in facilitation of required activities such as the MFC Plan of Care, equipment, medications, and supplies.
  • Conduct admission activities and post‑admission home visits to ensure quality and safety.
  • Provide ongoing medical social work support and follow‑up for MFC parent, including technical assistance.
  • Accompany MFC providers and children/youth to appointments and coordinate Medicaid enrollment processes.
  • Document activities in electronic documentation system.
  • Provide after‑hours on‑call program support as needed.
Quality Assurance
  • Maintain, review, and monitor MFC related data and EHR entries.
  • Verify accuracy and quality of data, assessments, and documentation entries.
  • Evaluate MFC parent skills, competencies, and documentation timelines with stakeholders.
  • Develop and monitor technical assistance plans for quality improvement.
  • Implement organization, program, and individual quality assurance standards.
  • Participate in CMS training and related follow‑up activities.
  • Provide reports from database systems as requested by management.
Other Duties As Assigned
  • Coverage of CMAT SW duties as assigned.
