LTSS Service Coordinator – Case Manager (Bilingual in Spanish/English)

Elevance Health is seeking an LTSS Service Coordinator, also known as a Case Manager, to conduct service coordination functions for a defined caseload of individuals in specialized programs.

Primary field‑based position. Candidate must reside in Miami‑Dade, Broward, Lee, Hillsborough, or Palm Beach County.

Schedule:

Monday‑Friday, 8 a.m.‑5 p.m.

• Perform face‑to‑face program assessments using predefined tools; apply motivational interviewing techniques to evaluate and manage individuals’ waivers (LTSS/IDD) and BH or PH needs.
• Identify members with potential clinical health‑care needs and coordinate care with the clinical healthcare management and interdisciplinary team.
• Manage non‑clinical needs of members with chronic illnesses or disabilities to ensure cost‑effective utilization of long‑term services.
• Document short and long‑term service and support goals in collaboration with the member’s care team.
• Identify members who may benefit from alternative service levels or other waiver programs.
• Act as mentor or preceptor for new staff and support process improvement initiatives.
• Submit utilization/authorization requests to utilization management with supporting documentation.
• Report critical incidents to state and county agencies, including Adult Protective Services and Law Enforcement.
• Assist with appeals, grievances, and state audits.

• B.A./B.S. degree and at least 2 years of experience working with a social work agency, or an equivalent combination of education and experience.
• Specific education, years, and type of experience may be required based on state law and contract requirements.

• B.A./B.S. degree in a health‑care related field.
• Bilingual in English and Spanish.
• Experience with LTSS, case management, social work, or hospital discharge planning.
