Traveler Requirements:
Must have home health experience; OASIS training preferred. Certs required: BLS, state licensed. Must have 1 year experience. Must have valid driver's license and car. Must be COVID vaccinated or have exemption on file. Must have flu shot or declination and wear mask at all times while at work.
Responsibilities:
- Provide home health therapy and rehabilitation services to patients in their residences.
- Assess patient needs and develop individualized treatment plans.
- Educate patients and their families on medication management, safety, and injury prevention.
- Document care and progress using Bay Care Forms, Bay Care Scheduling, and Ace Timesheet systems.
- Adhere to safety guidelines and maintain compliance with all healthcare regulations.
