Nurse Practitioner / Physician Assistant - Primary Care

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and provide team-based primary care to patients residing in Assisted Living and Memory Care communities in and around South Tampa, FL.

Our model of care is preventative, proactive and tailored to patients  needs, with multidisciplinary teams including patients, families, and other providers.

Bluestone patients experienced 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions compared to similar patients over the same period.

Position Overview:

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday–Friday with admin time built into schedule
• Manage a panel of approximately 200 patients (roughly 12–16 patients per day, 14 on average)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants, RNs, LPNs, Social Workers and care navigators
• Collaborate with community partners in home care, hospice, and other resources as appropriate

Qualifications:

• Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
• Board Certification/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or able to obtain licensure through the Florida Board of Nursing or Board of Medical Practice
• Ability to obtain and/or maintain DEA certification for the State of Florida
• Strong verbal communication and interpersonal skills
• Valid and unrestricted driver’s license; local travel required

Preferred Qualifications:

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
