• Oversee day-to-day operations of a clinical interdisciplinary team
• Ensure quality and appropriate care delivery and staffing
• Act as liaison to resolve patient care issues and communicate with physicians
• Supervise, mentor, and train clinical staff in collaboration with other leaders
• Support clinical program oversight and ensure compliance with agency goals

• Graduate of an accredited nursing program; must be a licensed Therapist or Registered Nurse (RN, PT, OT, or SLP)
• 1+ year of Medicare Home Health field experience; experienced field clinicians encouraged to apply
• Strong judgement, leadership, and problem‑sol skills
• Ability to motivate and lead a team to achieve common goals
