- BE/BC in Family Medicine, Internal Medicine, or Geriatrics - REQUIRED
- Geriatric fellowship training - Preferred, not required
- Join a large multispecialty practice with 32 providers and 12 specialties/subspecialties
- Split schedule:
- 2.5 days in outpatient clinic
- 2 days providing nursing home care
- Work within a Patient-Centered Medical Home
- NHSC-approved site (loan repayment eligible)
- Team includes Physician, NP, and RN
- EMR: EPIC (system-wide integration)
- Employment through physician group
- Full spectrum of clinic services available:
- Primary & preventive care
- Immediate care
- Specialty services
- Lab, X-ray, ultrasound, ECHO
- Strong built-in referral network
- Clinic located adjacent to the hospital
