Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in‑house support staff. We are seeking a licensed mental health provider (LCSW, LISW, LPCC, LCMHC, LMFT) to provide recovery‑oriented rehabilitation services in group or individual settings for residents in long‑term care, addressing a wide range of issues from anxiety and depression to severe mental illness and end‑of‑life concerns.
Essential Duties and Responsibilities- Assess each resident with valid consent to determine individual needs and develop a treatment plan that meets those needs, ensuring all care is medically necessary.
- Perform written or verbal consultations with the resident’s primary care physician, family, facility staff, and Eventus Whole Health support staff as needed.
- Document treatment plans, therapy sessions, and updates according to Eventus Whole Health’s procedures and policies, and provide facility documentation within 48 hours of service.
- Meet with the Resident Care Coordinator or designated staff upon arrival and departure to review current care needs and communicate findings.
- Maintain open and positive communication with facility staff and administration, providing written or verbal instruction on treatment plans and caregiver information.
- Adhere to Eventus Whole Health standards, policies, and procedures, and comply with all legal and regulatory agencies’ rules, guidelines, and professional ethical standards.
- Conduct yourself professionally at all times.
- Maintain patient confidentiality, including appropriate use of cell phone, email, text, patient records, and EMR, and abide by HIPAA rules.
- Consult with the clinical supervisor when faced with an unfamiliar resident or request.
- Psychologists:
Doctoral degree from an accredited institution and current state license as a doctoral‑level psychologist and health services provider. - Therapists:
Licensed Clinical Social Worker, Licensed Independent Social Worker, Licensed Professional Clinical Counselor, Licensed Clinical Mental Health Counselor, or Licensed Marriage and Family Therapist;
Master’s degree in social work and a current state license.
- Assessment techniques, psychotherapy techniques, population‑specific interventions, and implementation of treatment plans.
- Use of diagnostic tools, including the current DSM edition.
- Rules and regulations governing psychotherapy practice.
- Organization policies and procedures.
- Gathering and analyzing physiological, socio‑economic, behavioral, and emotional patient data.
- Accurately evaluating patient problems and providing appropriate advice, intervention, or referral.
- Written and verbal communication.
- High degree of initiative, judgment, and discretion.
- Establishing and maintaining effective working relationships.
- React calmly and effectively in emergency situations.
- Work collaboratively with all members of the health‑care team.
- Make responsible decisions within scope of practice.
- Evaluate and recommend continuous quality improvement.
- Handle confidential and sensitive information.
