Physical Therapist; PT - Home Health

BAYADA Home Health Care seeks a full‑time Physical Therapist (PT) to perform home health visits in Greenville, NC, serving adults and geriatric clients across Wilson, Nash, Edgecombe, and Pitt counties.

• Make home visits within assigned geographic territories.
• Perform diagnostic tests and measurements, including joint mobility, transfer status, stability, ambulation patterns, strength, endurance, and balance testing.
• Implement wound care interventions utilizing evidence‑based dressings and topical agents.
• Develop and implement individualized care plans with manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the PT care plan and collaborate with nursing as part of a multidisciplinary team.
• Educate clients, families, or representatives on rehabilitative care to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a touch‑pad tablet.

• Current North Carolina Physical Therapist license.
• Graduated from an APTA‑approved program or AMDA‑approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; experience with electronic medical records preferred.
