Position: Case Manager II, Registered Nurse (RN) - Freedom Hill Community Health Center

Case Manager II, Registered Nurse (RN) – Freedom Hill Community Health Center

Location:

Greenville, NC

Schedule:

Monday, Tuesday, Thursday, Friday – 8:00 AM to 5:00 PM;
Wednesday – 11:00 AM to 8:00 PM. 8‑hour day shift; 8‑hour evening shift on Wednesday.

Position Overview

Provide medical case management services to a specified panel of patients within the clinical setting. 40‑hour work week, no weekends, full benefits package, 12 paid holidays, bi‑annual bonus, bi‑weekly pay.

Essential Tasks
  • Provide medical case management to medium/high‑risk patients.
  • Perform annual comprehensive assessments to evaluate patients’ health‑care needs and social determinants of health and develop individualized, person‑centered care plans with patient and family participation where possible.
  • Collaborate with community agencies for referral resources and use NC Care 360.
  • Establish, document, and update care plans as patients’ needs change or gaps in care arise.
  • Identify patients with recent ED or hospital admissions and assist transition back to primary care, including medication reconciliation and access.
Experience and Education
  • Graduate of an accredited school of nursing as a Registered Nurse.
  • Current and unrestricted license to practice in North Carolina.
  • Preferred previous experience in a community health center setting, nursing, or case management.
  • Demonstrated skills and judgment to provide direct care under provider supervision.
  • Strong computer skills and working knowledge of Windows and an Electronic Health Record system.
  • Active North Carolina Driver’s license and personal vehicle available for frequent travel.
  • Effective communication and interpersonal skills.
Physical Requirements
  • Constant standing and walking; occasional bending, stooping, lifting, reaching forward and overhead.
  • May occasionally lift patients or equipment, usually not exceeding fifty pounds.
  • Frequent computer work.

Base Pay Overview

Starting at $31.50 per hour; actual salary may vary based on qualifications, skills, competencies, and proficiency.
