Clinical Pharmacist
We are seeking an experienced Lead Clinical Pharmacist to develop and expand our Primary Care Network pharmacy team. The post holder will work across the two GP practices delivering advanced clinical medicines optimisation, structured medication reviews, and medicines safety leadership. This is a senior, patient-facing role requiring autonomous practice as an independent prescriber, with responsibility for complex patients, service development, quality improvement and leadership within the multidisciplinary team.
Mainduties of the job
Deliver face-to-face and remote structured medication reviews, focusing on polypharmacy multimorbidity care home residents and vulnerable patients
Act as an independent prescriber, implementing prescribing changes and ordering appropriate monitoring
Lead repeat prescribing systems, reauthorisations and medicines optimisation on transfer of care
Manage medicines reconciliation following hospital discharge and reduce medicines-related admissions
Lead on medicines safety and governance
Drive and deliver quality improvement audit QOF and CQC medicines-related activity
Support population health initiatives and cost-effective prescribing programmes
Provide expert medicines advice and education to GPs MDT colleagues and patients
Support our Pharmacy Technicians and the wider pharmacy team
Work collaboratively with PCN leadership ICB pharmacists and secondary care teams
About usThis is an exciting opportunity for a dynamic, committed pharmacist to lead, shape and develop our pharmacy team in a progressive, supportive, forward thinking Primary Care Network.
Our PCN is formed of 2 practices, Wellington Medical Centre and Lister House Surgery. Wellington Medical Centre is a partnership of 9 GP partners, 7 salaried GPs and a retained GP looking after approximately 23,000 patients. Lister House is part of Symphony Healthcare Services, located in Wiveliscombe serving a rural population of around 6,800 patients. In both surgeries you will work alongside a well-established, friendly, multidisciplinary team of GPs, allied health professionals, pharmacy technicians and administrators.

The following is not an exhaustive list and the post holder may at times need to undertake duties as per the needs of the PCN
Lead DutiesLead a team of Clinical Pharmacists and Pharmacy Technicians and demonstrate the characteristics of a role model to members in the team providing support to team members and ensure there is appropriate clinical supervision in place for all pharmacists
Undertake clinical supervision and systematic peer caseload review of colleagues on an individual or group basis
Ensure all pharmacists are engaged with their review and appraisal systems within the PCN
Act as a role model demonstrating effective clinical leadership on a daily basis and promote the behaviours of being proactive, positive, respectful, supportive, reliable and trustworthy
Conduct teaching and assessment effectively according to a learning plan with supervision from a more experienced colleague
Understand the implications of national priorities for the team and/or service and manage the team of Clinical Pharmacists through these changes
Assess and monitor risk in own and others practice acting on results thereby ensuring safe delivery of care
Follow professional and organisational policies and procedures relating to performance management
Participate in maintaining systems for risk assessment and minimisation including promoting a fair blame culture in order that near misses and incidents are reported and investigated
Contribute to investigation of incidents and complaints when required; participate in identifying lessons learnt and the sharing of learning across the organisation
Ensure dignity, privacy and cultural and religious beliefs are respected at all times
Continually monitor standards of care through quality markers achievement of PCN targets and benchmarking
Be aware of local quality data and use it to identify practice development required
Work proactively in managing change in own speciality to improve practice and health outcomes
Identify and provide leadership on areas of prescribing and medicines optimisation Conduct clinical audits and improvement projects or work with colleagues such as PCN staff Practice staff etc
Present results and provide leadership on suggested changes
Contribute to national and local research initiatives
Clinical Responsibilities- Conduct patient-facing long-term condition clinics eg COPD asthma ensuring medicine optimisation and ongoing medication monitoring
- Undertake structured clinical medication reviews for patients with polypharmacy multiple comorbidities or specific clinical needs
