Pay Rate: £44.00 per hour (Umbrella) / £32.13 per hour (PAYE)
Working Pattern:
Hybrid Working – 1 day per week in the office (Wednesday at County Hall, Taunton)

• Provide professional leadership and support within Adult Social Care services.
• Promote wellbeing, independence, and person-centred practice in line with the Care Act.
• Complete timely and robust assessments, reviews, and support plans.
• Safeguard vulnerable adults and manage complex safeguarding enquiries.
• Support individuals to remain independent within their communities for as long as possible.
• Work collaboratively with health professionals, families, carers, and partner agencies.
• Facilitate safe and timely hospital discharges and help prevent unnecessary admissions.
• Ensure services enhance existing informal support networks and community resources.
• Support people to achieve outcomes that are important to them while maintaining choice and control over their care and support.

• Qualified Social Worker.
• Registered with Social Work England.
• Significant experience working within Adult Social Care.
• Strong knowledge of safeguarding legislation, policy, and practice.
• Experience managing complex cases and undertaking risk assessments.
• Excellent communication, decision-making, and leadership skills.

• Promoting wellbeing and independence.
• Reducing or delaying the need for long-term support.
• Enabling people to achieve their desired outcomes.
• Delivering personalised assessments and reviews.
• Safeguarding vulnerable adults in all aspects of practice.

PAYE: £32.13 per hour
Umbrella: £44.00 per hour
