Under general supervision of the nursing supervisor, utilizes the knowledge and application of nursing principles to independently provide direct patient care in the home. Assesses need for allied health services (home health aide, physical therapy, occupational therapy, dietetics, speech‑language pathology, social work) to ensure integrated home health delivery. Performs related duties and projects as assigned. Mileage reimbursement at IRS rate.

• Graduation from an accredited Registered Nurse program.
• Current licensure by the State of Michigan as a registered nurse.
• Minimum 9 months to 1 year clinical nursing experience preferred.
• Strong communication, interpersonal and customer service skills.
• Proficient in computer use for electronic documentation.
• Current driver's license, satisfactory driving record, reliable transportation.
• Ability to lift, carry, transport and move clients when needed.
