JOB DESCRIPTION:The Utah Department of Health & Human Services, Division of Correctional Health Services, is seeking a Care Navigator (Caseworker II) to join our team of skilled professionals.
We provide evidence-based, individualized interventions to safely stabilize a wide range of mental health issues and help individuals maintain their best level of functioning while in custody.
Our goal is to develop and carry out high quality, evidence based programs aimed at improving individual outcomes and enhancing community safety by implementing strategies, promoting accountability, and strengthening partnerships while maintaining a positive work environment.
IDEAL CANDIDATE:- Licensure as a Social Service Worker or Certified Substance Use Disorder Counselor or working towards getting their licensure.
- My underfill as a certified case manager.
- Experience working with substance use disorder and/or mental health treatment.
- Experience teaching educational classes.
- Experience in case management principles.
- Effective speaking, reading, and written communication skills.
- Knowledge of risk assessments and criminogenic risk factors.
- Familiar with medication treatment for opioid use and other substance use disorders.
These are examples of an ideal candidate but do not preclude other candidates from being considered.
PREFERENCE:- May be given to candidates who hold current Licensure as a Social Service Worker or Certified Substance Use Disorder Counselor or are working towards getting their licensure.

If offered this position, your employment will be contingent upon passing a background check and drug testing. There will be no cost to you for these checks. This check will include fingerprinting, which will be available at various DHHS locations for your convenience and drug testing at a facility close to your location. Fingerprinting will be completed prior to your first day of employment.
- Coordinates efforts to find appropriate services (housing, transportation, employment, medical, mental health and other social services) from community providers, collateral agencies and other community partners.
- Provides psycho-educational classes to address criminogenic risks and counseling for offenders with substance use disorders and mental health disorders. Services may also be provided to family members.
- Helps clients obtain necessary identification (i.e birth certificate, drivers license, tribal , etc.)
- Provides written and verbal communication with Adult Probation and Parole Agents and Supervisor on treatment progression.
- Provide case management services, coordinate, advocate, monitor and link clients to appropriate services in the facility and outside in the community if discharged.
- Other duties as required.
