The Lead Nurse is responsible for the leadership and development of the nursing team across our six-site, 22,500-patient practice. The team consists of 6 Practice Nurses, 2 Nurse Associates, and 8 Healthcare Assistants. The post-holder will provide professional and clinical leadership for the team, working in partnership with the Practice People Manager to ensure effective staff management. Whilst the Lead Nurse will lead on clinical practice, professional standards, and service delivery, the Practice People Manager will provide support with all HR-related aspects of team management, including recruitment, performance management, employee relations, and workforce development.

As a senior member of the multidisciplinary team, the post-holder will combine clinical expertise with strong leadership to ensure high-quality, evidence-based nursing services. They will oversee chronic disease management, vaccination programmes, health promotion, policy development, and quality assurance, ensuring safe and effective patient care.

The Lead Nurse will contribute to Infection Prevention & Control (IPC) processes (without holding the IPC Lead role), support clinical governance activities, and work collaboratively with GPs, managers, and external partners. The post-holder will act as a clinical role model and mentor, supporting the development of the team and helping to embed continuous quality improvement across the organisation.

Channel View Medical Group is a multidi­isciplinary team, led by 5 GP Partners and one managing Partner, with sites at Teignmouth, Shaldon, Bishopsteignton and Chudleigh.

Primary responsibilities

The following are the core responsibilities of the Lead Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

• Assess the health needs of patients and ensure care is clinically appropriate, evidence based, and in line with current guidelines.
• Develop, implement, and evaluate treatment and management plans for patients with chronic diseases, promoting health and wellbeing.
• Identify, manage, and support patients at risk of developing long term conditions, aiming to prevent deterioration and unnecessary complications.
• Provide routine and urgent nursing care in accordance with NICE guidance, National Service Frameworks, and local protocols.
• Diagnose and manage both acute and chronic conditions within the scope of professional practice.
• Prescribe as a Non-Medical Prescriber (NMP) if qualified, or undertake the qualification with practice support.
