Social Worker – Central Adults Community Team (Shropshire) – £28.40 per hour – Full-time, Ongoing Contract

What you will do:

• Conduct comprehensive face-to-face assessments of individuals’ needs in the Central Shropshire community.
• Develop and implement personalised care plans that promote wellbeing and independence.
• Coordinate with multidisciplinary teams to provide holistic support tailored to each service user’s requirements.
• Advise and assist service users and their families, directing them to appropriate services and resources.
• Ensure compliance with legal and organisational policies, maintaining accurate and up-to-date records.

Requirements of the Social Worker:

• Hold a degree or equivalent in social work with current registration with Social Work England.
• Possess significant frontline experience in Social Services within the UK as a qualified Social Worker.
• Demonstrate excellent interpersonal skills and the ability to adapt to various situations effectively.
• Be a licensed driver to facilitate travel for assessments across Central Shropshire.
