As we prepare to reopen our Shrewsbury ward following a £3 million makeover there is a range of inpatient and community roles available.
We are keen to attract both newly qualified individuals at the beginning of their careersand experienced clinicians.

Healthcare Assistants to join our Inpatient or Community Nursing Team.
With your friendly, empatheticand professional approach, you will work in a highly rewarding area of care,supporting patients under the direction of a Registered Nurse or Nurse Practitioner maintaining a consistently high standard of person-centred palliative care.
As a valued member of our skilled and caring clinical team, you will play a vital role in meeting patientsholistic needs, ensuring comfort, safety and dignity, while also offeringongoing support to families and carers.
If the community team is moreyour area, as a valued member of our skilled and caring team, you will contribute to a range of services including home-based care, coffee morning sand supporting outpatient clinics.
Main duties of the jobTo assist in the care of patients with a life limiting illness under the direction of a Registered Nurse and to maintain a high standard of care. To support the patients and relatives through ongoing communication
To generate and maintain a friendly team spirit with all the staff and volunteersof the Hospice.
Toprovide a safe environment for patients, visitors, staff and volunteers.
Tosupport new HCAs into their duties
Ifrequested, to provide assistance on Hospice transport, ensuring safe transferof patients to and from the Day Services.
Tohelp embed the practices and culture of rehabilitative palliative care to effectively support people with long term life-limiting conditions to livewell.
Community Based:
Our primary goal is to establish a tiered hospice communitycare model that can respond promptly to support our patients with theirevolving, complex needs, enabling them to remain safely at home if that is theirchoice.
Healthcare Assistants will support the Service through H@H day care, coffee mornings, and supporting outpatient clinics, to ensure that high standards of care are delivered and effective liaison and communication with key professionals involved in the patients care are maintained.
Individuals will need to demonstrate the ability to work both independently and as part of amultidisciplinary team. They will needto be flexible in their approach to the variety of tasks they will undertake.

Job responsibilities PRINCIPAL RESPONSIBILITIESTo support the trained members of the nursing team in the implementation of individualised care programmes for patientscomfort, safety and dignity at all times (e.g. checking drugs with trainedmembers of staff/working within current risk assessments for individualpatients)
To perform designated nursing tasks when deemedcompetent theoretically and practically by a qualified nurse with the Mentor qualification, as part of a planned programme of care (e.g.changing dressings, catheters, observing and documenting, testing urine,care of people using prescribed oxygen).
To report to the Ward Sister/ Ward Manager as appropriate for any changes in patients condition or relevant informationpertinent to patient care (e.g. dietary requirements).
To monitor and record observations (e.g.Temperature, Pulse, Respiration, Blood Sugar and Blood Pressure) oncompletion of relevant competency training.
