7‑day week rota; shift pattern can be 3 or 4 long days or 5 shorter days; block booking only.

Key Responsibilities- Deliver high‑quality nursing care in the community, empowering patients to maintain independence.
- Assess, plan, implement, and evaluate person‑centred care plans.
- Collaborate with multidisciplinary teams, GP surgeries, and acute trusts.
- Assist in facilitating smooth hospital discharges and reducing readmissions.
- Perform clinical skills including:
- Complex wound care including Doppler compression
- Simple wound care
- End‑of‑life care
- Medication administration
- Catheterisation (male, female, suprapubic)
- Bowel care
- Syringe driver
- Insulin administration
- Doppler assessment
- Compression bandaging
- NMC registration (valid and active).
- A minimum of 6 months NHS experience as a Community Nurse.
- Eligibility to work in the UK (no consideration for applicants without valid UK work eligibility).
- Competitive rates of pay: £15.18 – £32.00 per hour.
