Enhanced Closing Date: 06/09/2026 at 23:00 Reference: 711905 We’re excited to offer a fantastic opportunity for a qualified Social Worker to join our dedicated Adult Social Care Team here at Wokingham Borough Council, on a full-time, permanent basis. As a Social Worker, you will work with a diverse range of individuals supporting them to live the life they choose. You’ll focus on what individuals can do, not just what support they need whilst getting to know the people you work with and what matters most to them.

The work you do will enable carers to continue in their caring roles for as long as they choose and are able to. This is an exciting opportunity to work differently alongside GPs, social prescribers, and voluntary sector partners, helping people get the right support at the right time, and preventing needs from escalating. You’ll build strong relationships across health and social care, bringing a social work perspective into primary care and supporting people to live the lives they choose.

You will work within a Primary Care Network (PCN) providing social work expertise within primary care, supporting patients with complex social, emotional, and safeguarding needs. The role aims to:
Improve health and wellbeing outcomes, reduce health inequalities, prevent hospital admissions. Support integrated community-based care. Typical Outcomes of the Role:
• Reduced GP workload through appropriate social interventions
• Improved patient independence and wellbeing
• Fewer unnecessary hospital admissions
• Improved safeguarding outcomes
• Enhanced integration between health and social care

• Based across GP practices within a PCN
• Community-based work (home visits, outreach)
