Opus People Solutions are recruiting on behalf of Telford & Wrekin Council for an experienced Senior Social Worker to join the TICAT Hospital Team. This is an excellent opportunity for a skilled practitioner to play a vital role in supporting safe, effective, and timely hospital discharges while delivering high-quality outcomes for adults and their families.
Working within a fast-paced multidisciplinary environment, you will manage complex cases, undertake statutory assessments, and provide professional leadership to colleagues while ensuring compliance with legislative requirements and best practice standards.

Key Responsibilities

Manage a complex caseload of adults with a range of physical health, mental health, learning disability, and social care needs.
Undertake Care Act Assessments, reviews, and person-centred support planning.
Complete Mental Capacity Act Assessments and facilitate Best Interest Decisions where required.
Conduct safeguarding enquiries and risk assessments in accordance with relevant legislation and local procedures.
Support and facilitate safe and timely hospital discharges, reducing delays and promoting independence.
Work collaboratively with health professionals, therapists, discharge coordinators, ward staff, families, and partner agencies.
Promote strengths-based practice and maximise service users' independence and wellbeing.
Provide guidance and support to less experienced practitioners within the team.
Maintain accurate and timely case recordings and reports.
About You
To be successful in this role, you will have:

Qualified Social Worker status (Degree/DipSW/CQSW or equivalent).
Current registration with Social Work England.
Significant experience within Adult Social Care, ideally within a hospital discharge or acute setting.
Sound knowledge of the Care Act 2014, Mental Capacity Act, safeguarding legislation, and relevant social care frameworks.
Excellent assessment, risk management, and case management skills.
The ability to work effectively within a multidisciplinary team.
A full UK driving licence and access to a vehicle.
