Home Health Physical Therapist

Angelic are Home Health Inc is looking for a passionate and dedicated Home Health Physical Therapist to join our growing team in Temecula, CA. In this rewarding role, you'll make a meaningful difference by delivering personalized, one-on-one physical therapy care to patients in the comfort of their own homes. • Conduct comprehensive in-home physical therapy evaluations and develop individualized treatment plans
• Provide skilled PT interventions to improve patient mobility, strength, balance, and functional independence
• Monitor and document patient progress, adjusting care plans as clinically appropriate
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Collaborate with the interdisciplinary care team including nurses, OTs, and physicians to ensure coordinated care
• Maintain accurate and timely clinical documentation in compliance with Medicare and state regulations
• Participate in care conferences and contribute to discharge planning

• Active California Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Home health or outpatient PT experience preferred
• Knowledge of Medicare home health guidelines and OASIS documentation
• Valid driver's license, reliable transportation, and current auto insurance
• Strong clinical assessment, critical thinking, and patient communication skills
• Ability to work independently and manage a flexible caseload in the field
