Family Medicine Nurse Practitioners (FNP) will provide high-quality medical care to a wide variety of patients in a comprehensive, quality-oriented, and supportive manner at Mountain Park Health Center. Your role will encompass a broad range of primary care services, focusing on preventive care, health maintenance, the management of acute and chronic medical conditions, and follow‑up services to patients under your care.

• Provides direct patient care by obtaining medical history, performing a physical examination, and creating an assessment and plan that may include ordering and interpreting labs, ordering imaging studies, and developing a treatment plan including referring to specialty care when needed.
• Administers or prescribes treatments and medications.
• Follows up and monitors patient’s progress throughout the treatment plan.
• Documents the evaluation and plan of care.
• Provides patient education on a variety of health topics.
• Collaborates and consults with other in‑house allied health professionals to ensure coordinated patient care.
• Maintains regular and predictable attendance.
• Performs other duties as assigned.

• Approved Nurse Practitioner education program, experience in clinical area of specialty.
• Valid Arizona Registered Nurse License.
• Valid Arizona Nurse Practitioner certificate.
• Valid CPR certificate.

• 1 year of relevant experience in Family Medicine or Geriatrics.
