Summary

We are seeking a full‑time Physician to serve as the Home‑Based Primary Care (HBPC) Medical Director. The role provides clinical oversight of all non‑institutional care programs and collaborates with the HBPC Program Manager. A recruitment/relocation incentive may be authorized for a highly qualified candidate.

Qualifications
  • United States citizenship (non‑citizens only if no qualified citizens can be recruited).
  • Doctor of Medicine or equivalent (allopathic/osteopathic) degree accredited at time of completion.
  • Current, full and unrestricted license to practice medicine or surgery in a U.S. state, territory, or the District of Columbia.
  • Completed residency in a VA‑approved core specialty leading to board‑certification eligibility (ACGME or AOA, or equivalent U.S. program with ≥5 years practice).
  • Physicians in accredited residency programs who meet basic requirements may be appointed as Physician Resident Providers (PRPs) on an intermittent or fee basis.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics (preferred).
  • 3‑5 years experience in Geriatrics or Palliative Medicine, 1‑2 years supervising NPs, and 2‑3 years supervisory experience in a VA or community setting.
  • Proficiency in spoken and written English per 38 U.S.C. §§ 7402(d) & 7407(d).
  • Physical ability to lift 15‑44 lb, walk 4‑6 h, stand 4‑6 h, use fingers, and meet vision/hearing requirements listed below.
  • Preferred:
    Board certification in Internal Medicine/Family Medicine and Geriatrics or Palliative Medicine.
  • Physical requirements:

    Moderate lifting (15‑44 lb), use of fingers, walking (4‑6 h), standing (4‑6 h), near vision 13‑16″, far vision 20/20‑20/40, color vision, hearing (aid permitted).
Duties & Responsibilities
  • Serve as senior clinician leader of the HBPC team; provide overall medical care and collaborate with the Program Director.
  • Possibility of faculty appointment and involvement in academic activities with medical schools.
  • Provide direct patient care and/or supervise licensed independent practitioners (residents, fellows, medical students) in home settings.
  • Assume administrative leadership of HBPC development, including accreditation processes and program improvement plans.
  • Determine appropriate medical interventions for patients; initiate/adjust therapy, order diagnostics, prescribe medications per VA policy.
  • Collaborate with in‑house specialty clinics and community providers; serve as primary medical responsibility for HBPC patients.
  • Advocate for HBPC with VHA leadership and the medical community.
  • Select HBPC team members and provide clinical oversight of treatment plans.
  • Be available to team members for collaboration when medical or other problems arise; participate in interdisciplinary team meetings.
  • Maintain educational activities to keep the team apprised of medical advances; sign physician‑required forms; possess DEA licensure for controlled substances II‑V.
  • Coordinate admissions, consults, and hospital referrals as needed.
  • Collate and direct HBPC patient visits when appropriate.
  • Collaborate with community home‑health and hospice agencies; follow safety and infection‑control procedures.
  • Comply with Medical Center, VHA regulations, and use of government vehicles.
Work Schedule

Monday‑Friday, 8:00 a.m. – 4:30 p.m.

Compensation & Benefits
