Temple Rehabilitation Hospital is a 36-bed inpatient acute rehabilitation facility located at 23621 SE H.K. Dodgen Loop in Temple, Texas. It is designed to help individuals recover from serious injuries or illnesses that have significantly impacted their ability to function independently. The facility is dedicated to making communities healthier.

• Tailored care for patients recovering from stroke, brain injury, spinal cord injury, amputation, orthopedic trauma, and other neurological or complex conditions.
• Intensive therapy model:
  Patients receive at least three hours of physical, occupational, and/or speech therapy per day, five days a week, under the supervision of a rehabilitation physician.
• State-of-the-art facility with modern therapy equipment and private rooms.

As a Case Manager, you will coordinate the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting. You will serve as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.

• Strong communication and coordination skills required.
• Ability to manage complex cases and work collaboratively with medical and non-medical staff.

• Education & License: Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Certifications: Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Experience: Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute rehabilitation hospital experience strongly preferred.
• Skills: Effective oral and written communication in English; additional languages preferred. Proficiency in Microsoft Office applications required.
