Charge Nurse-LVN
STGi is currently seeking a Charge Nurse (LVN) to provide services at The William R. Courtney Texas State Veterans Home.

Job Summary JOB SUMMARY:
To contribute to the success of the community by providing quality nursing care to residents and to coordinate total nursing care for residents by implementing specific procedures/programs and being in compliance with all STGi policies and procedures, state regulations and federal regulations.
Essential Functions
Follow established standards of nursing practices and implement STGi programs, policies and proceduresSupervise and evaluate all direct resident care and initiate corrective action as necessary.Obtain report from nurse being relieved and record sufficient information to implement appropriate follow-up action as necessary.Provide report to nurse coming on duty, including sufficient information for follow-up action as necessaryTake appropriate action to provide staffing to meet residents' needsConduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective actionIdentify safety hazards and initiate corrective action immediatelyIdentify resident problems and emergency situation and initiate emergency care per physician's orders in the absence of a physicianProvide supervision to Nursing Assistant providing direct resident careAttend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in-service classes and committee meetings as assignedReview the Glucometer Control Log to monitor the machine was checked that day prior to testing for blood sugar levelsFollow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident populationCounsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service, when necessaryComply with community processes related to pain assessment/management/documentation, providing treatments/documentation and delivering medications/documentationAdminister and document direct resident care, medications and treatments per physicians' orders and accurately record all care providedProvide instruction for appropriate nursing care to licensed nurses and Nursing AssistantsDocument assessment of residents as assigned as per your licensure. Report untoward findings to physician and your supervisorObserve the 5 RIGHTS of medication/treatment administrationPromptly report changes in residents' conditions to the physician, Director of Nursing Service and responsible party, and take follow-up action as necessary. Document notifications in resident's clinical record.Follow Community admission procedures and communicate resident admission to other departments, including rehabilitationCompetently perform basic nursing skills as per your licensureFollow Community discharge /transfer procedure/documentation and coordinate discharge/transfer with other departments, physicians and ancillary service providersSupervise response to resident's call for assistanceListen to resident, family, or physician concerns and initiate corrective action and/or report concerns to Nursing Supervisor.Count narcotics, check emergency kit and exchange keys each shiftCheck meal trays and nourishment to monitor proper diet is served and residents are assisted as necessaryOrder, receive and store medications appropriatelyObserve community process related to signing out narcotic and administration thereof. This includes the process related to shift narcotic count and documentation thereofNotify physician, when necessary, of changes in residents' conditions, weight, medication errors, medication not available, abnormal laboratory/X-Ray results and any other unusual observationsDocument daily notes on unstable residents/change in resident condition, Medicare residents and recently admitted residentsBe responsible for the safety of residents and team members under his/her supervisionObserve infection control proceduresPerform all the duties of a certified nursing assistant when requiredRequired Skills
Must have a current and valid LVN nursing license in the state of Texas, or a current and valid LVN/LPN license from a Compact Party State.
Required Experience
REQUIRED
EDUCATION:
Must have a current and valid LVN nursing license in the state of Texas, or a current and valid LVN/LPN license from a Compact Party State.
Current Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification required prior to performing work, preferably from American Heart Association (AHA), or have the ability to obtain certification within thirty (30) days of employment.
