This role is part of a modern, patient‑focused team of clinicians dedicated to helping patients recover and regain independence from the comfort of their homes.
Base Pay Range$70,500 – $96,900 per year.
Responsibilities- Provide admission, case management, and follow‑up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient; provide necessary follow‑up as directed by the Clinical Manager.
- Consult with physicians to develop the initial plan of treatment based on orders and patient assessment; provide hands‑on care, manage and evaluate the care plan, and educate patients per physician orders under the Clinical Manager’s supervision; revise the plan in consultation with physicians as needed.
- Coordinate appropriate care among various healthcare personnel (physical therapists, occupational therapists, home health aides, external providers).
- Report patient care, condition, and progress to the patient’s physician and Clinical Manager continuously.
- Implement patient care plans with patients and families to aid in achieving optimal outcomes.
- Coordinate and supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers; provide written instructions and monitor documentation compliance.
- Discharge patients after consultation with the physician and Clinical Manager, completing all necessary clinical documentation.
- Prepare medical documentation for all patients, including case conferences, patient contacts, medication order changes, re‑certifications, progress updates, and care plan changes; submit visit/shift reports and summarize records with other health‑care disciplines.
- Diploma, Associate, or Bachelor’s Degree in Nursing.
- Minimum of one year of nursing experience (preferred).
- Strong experience in medical‑surgical, ICU, ER, or acute care environments.
- Experience in Home Health a plus.
- Current, unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
