Licensed Vocational Nurse (PACT) – U.S. Department of Veterans Affairs

SummaryThe primary purpose of the LVN in the Patient Aligned Care Team (PACT) is to perform assigned duties in a manner that promotes quality health care, provides customer service, and maintains an environment committed to serving the veteran. PACT is a team-based model of care featuring patient‑centered care, coordination of care, and access to care. This primary care model provides integrated and accessible health care with professionals practicing in the context of family and community.
Qualifications- Basic Requirements:
- Citizenship – Citizen of the United States. Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3 – section A – paragraph 3g.
- Education – Graduate of a school of practical or vocational nursing approved by the appropriate state agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed.
- Verification can be obtained from the state approving agency or the NLNAC – 61 Broadway, 33rd Floor, New York, NY 10006.

- Exceptions include:
- Health care education in the military service or training in the military service accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure.
- VHA employees who were converted to appointments under 38 U.S.C. 7401(3) and 38 U.S.C. 7405 in implementation of Public Law 98‑160 and Public Law 99‑576.
- Individuals granted a license by a jurisdiction that does not require graduation from an approved school.

- Licensure – Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a state, territory or Commonwealth (e.g., Puerto Rico) of the United States or District of Columbia.
- Physical Standards – See VA Directive and Handbook 5019.
- English Language Proficiency – Must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
- Grade Determinations:
- GS‑3:
None beyond the basic requirements. - GS‑4:
May be appointed or advanced to GS‑4 if any of the criteria in paragraphs 3b(2)(a) through (c) of the standard are met. - GS‑5:
Requires at least 1 year of qualifying experience at GS‑4 level or equivalent, and demonstrated knowledge and ability to provide a full range of practical nursing care. - GS‑6:
Requires at least 1 year of qualifying experience at GS‑5 level or equivalent; fully meets all performance requirements for the GS‑5 LPN/LVN.
- GS‑3:
- Preferred Experience:
Evidence of Lean Belt training or experience;
Charge Nurse/Preceptor experience;

This position includes a level of physicality normally associated with duties performed by nurses such as direct patient care, walking, standing, sitting, moderate lifting, repeated bending or kneeling. - Functional Statement #: 53052‑F. Permanent Change of Station (PCS):

- Provide advanced nursing care, including medication administration (including IV if certified), oxygen therapy, EKGs, lab draws, and bedside procedures.
- Document and report patient information, vital signs, medication reconciliation, and any significant deviations to RNs or physicians.
- Participate in interdisciplinary care planning, patient education, and family support within the PACT framework.
- Assist in entrusting responsibilities to Charge Nurses and manage mentorship of less experienced LPNs/LVNs and Nursing Assistants.
- Maintain compliance with all safety protocols, BCMA system usage, and facility policies.
- Work schedule:
Monday through Friday with shifts including 7:00am‑3:30pm; 7:30am‑4:00pm; 8:00am‑4:30pm, as needed by the clinic mission. - Recruitment Incentive –…
