Overview

STGi is currently seeking an LVN Charge Nurse (LVN) to provide services at The William R. Courtney Texas State Veterans Home. Our staff delivers compassionate and specialized care to our nation's veterans, focusing on the best experience for residents and their families.

JOB SUMMARY

To contribute to the success of the community by providing quality nursing care to residents and to coordinate total nursing care for residents by implementing specific procedures/programs and being in compliance with all STGi policies and procedures, state regulations, and federal regulations.

ESSENTIAL FUNCTIONS
  • Follow established standards of nursing practices and implement STGi programs, policies, and procedures.
  • Supervise and evaluate all direct resident care and initiate corrective action as necessary.
  • Obtain reports from nurses being relieved and record sufficient information to implement appropriate follow‑up action.
  • Provide reports to incoming nurses, including sufficient information for follow‑up action.
  • Take appropriate action to provide staffing to meet residents' needs.
  • Conduct resident rounds daily, reporting problems to the nursing supervisor and initiating corrective action.
  • Identify safety hazards and initiate corrective action immediately.
  • Identify resident problems and emergency situations and initiate emergency care per physician's orders in the absence of a physician.
  • Provide supervision to Nursing Assistants providing direct resident care.
  • Attend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in‑service classes, and committee meetings as assigned.
  • Review the Glucometer Control Log to monitor that the machine was checked that day prior to testing blood sugar levels.
  • Follow staffing guidelines and make nursing assignments according to staff qualifications and resident population characteristics.
  • Counsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service when necessary.
  • Comply with community processes related to pain assessment/management/documentation, providing treatments/documentation and delivering medications/documentation.
  • Administer and document direct resident care, medications, and treatments per physicians' orders and accurately record all care provided.
  • Provide instruction for appropriate nursing care to licensed nurses and Nursing Assistants.
  • Document assessment of residents as assigned per licensure; report untoward findings to physician and supervisor.
  • Observe the 5 Rights of medication/treatment administration.
  • Promptly report changes in residents' conditions to the physician, Director of Nursing Service, and responsible party, and take follow‑up action as necessary; document notifications in the resident's clinical record.
  • Follow Community admission procedures and communicate resident admission to other departments, including rehabilitation.
  • Competently perform basic nursing skills as per licensure.
  • Follow Community discharge/transfer procedures/documentation and coordinate discharge/transfer with other departments, physicians, and ancillary service providers.
  • Supervise response to a resident's call for assistance.
  • Listen to resident, family, or physician concerns and initiate corrective action or report concerns to Nursing Supervisor.
  • Count narcotics, check emergency kit, and exchange keys each shift.
  • Check meal trays and nourishment to monitor proper diet and assist residents as necessary.
  • Order, receive, and store medications appropriately.
  • Observe community processes related to signing out narcotics and their administration, including shift narcotic counts and documentation.
  • Notify physician of changes in residents' conditions, weight, medication errors, medication not available, abnormal laboratory/X‑Ray results, and any other unusual observations.
  • Document daily notes on unstable residents, changes in resident condition, Medicare residents, and recently admitted residents.
  • Be responsible for the safety of residents and team members under supervision.
  • Observe infection control procedures.
  • Perform all duties of a certified nursing assistant when required.
