Overview
Celina Health & Rehab, a skilled nursing and rehabilitation center located in Celina, TN near Dale Hollow Lake, is hiring a PRN Physical Therapist Assistant.
Job SummaryThe Physical Therapist Assistant will implement the plan of care established by the RPT and physician, maintaining quality of care.
Responsibilities- Screen all residents quarterly to identify functional declines and determine if Physical Therapy intervention is indicated.
- Carry out treatment program according to plan of care in consultation with Registered Physical Therapist.
- Identify need for modification of treatment plan and consult with Registered Physical Therapist.
- Monitor and report resident performance and progress.
- Assist Registered Physical Therapist in re‑assessment of patients and in determining need for continued or discontinued services.
- Assist Registered Physical Therapist in discharge planning.
- Supervise Rehab Therapy Aides.
- Educate staff, family, and caregivers as indicated.
- Identify needs for program development and implement programs per company policies.
- Identify equipment needs of Physical Therapy Dept. and order equipment per company and facility policy.
- Complete all required documentation per Documentation Manual.
- Coordinate in‑services for facility staff to ensure safe carry‑over of Maintenance/Restorative Programs.
- Attend all facility required meetings.
- Promote the profession within the facility, including coordinating special events during National Physical Therapy Month.
- Attend Continuing Education courses specific to the current position.
- Always carry out all job responsibilities in a professional manner.
- Associate degree in Physical Therapy Assistant program from an accredited institution.
- Certification and licensure according to state and national board requirements.
- Active state license required.
- Excellent organizational and time management skills.
- Excellent written and verbal communication skills.
- Current working knowledge of Functional Pathways policies, procedures, and clinical programs.
- Completion of all required screenings, certifications, and licensure as applicable.
