The Claiborne Women's Recovery Case Manager Duties:- The Recovery Home case manager will facilitate groups, provide weekly case management, and interface with community agencies and referral sources to coordinate care.
- Any other duties may be assigned as deemed appropriate.
- Serves as primary case manager that provides case coordination using a social model approach to a predetermined number of adult women.
- Interfaces with agencies to increase cooperation and consistency between work, community, behavioral, and physical health providers.
- Functions as a member of a Recovery team to plan, implement, and evaluate successful interventions for adult women.
- Embraces the key values of empowerment, normalization, rehabilitation, and continuity of care.
- The holder of this position will provide contacts with individuals that meet the requirements of the program and provide advocacy, linkage, and referral services as needed.
- Must maintain appropriate chart records and document case management and group meeting notes per agency standards.
- Must complete all documentation in a timely manner and according to agency standards.
- Must participate in direct supervision with program supervisor and director, as necessary.
- Must interface with community agencies and referral sources to coordinate care.
- Must provide Medication monitoring, therapeutic support and crisis intervention as needed.
- Must participate in weekly Recovery team meetings with relevant staff for coordination of care.
- Must uphold professional ethical standards and centers policy and procedures as well as CARF standards.
- Case manager may be required to work a flex shift based on the needs of the program participants.
- This position operates in the office, Recovery Home group rooms, and in the community.
- The Jail to Work and Recovery Home participants will attend groups in assigned rooms.

- Starting salary for this position is approximately $18.98/hr based on relevant experience and education.
- Due to the diversity of job responsibilities, this position requires flexibility in scheduling (days, evenings, and weekends).
- Must be capable of driving a van and also transporting clients in personal vehicle.
- Must also have the ability to communicate effectively and possess good time management and organizational skills.
- Must be computer literate and have ability to communicate effectively (oral and written communication.)
- One year experience in the social work and addictions field and/or behavioral health and criminal justice setting required.
- Additional experience utilizing trauma-informed practices with female clients who have experienced physical, sexual, or emotional abuse.
- Experience with addiction, motivational interviewing and re-entry programming preferred.
- Knowledge of Social Model Theory of Recovery Homes is preferred.
- Must have at least a Bachelor's degree and 1 year experience in a health-related field of counseling, psychology, social work, sociology, addictions, or a behavioral science field…
