• Review resident information and evaluate its impact on assessment and treatment.
• Conduct and document comprehensive functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable functional goals.
• Comply with evaluation, treatment, and documentation guidelines; maintain accurate records.
• Enter treatment and progress notes after each session.
• Revise treatment plans as needed based on resident progress.
• Document treatment course in clear, concise discharge summaries.
• Instruct assistants in treatment procedures and provide close supervision.
• Orient residents to program services and treatment procedures appropriate to their needs.
• Collaborate with nursing staff, residents, and families to maximize functional abilities.
• Instruct residents and caregivers on home exercises and therapeutic procedures.
• Assist in referral processes for additional services following discharge; maintain knowledge of community resources.
• Participate in rehabilitation team conferences, facility meetings, and quality improvement activities.
• Access continuing education opportunities to maintain clinical competency.
• Adhere to professional, regulatory, and facility standards.
• Maintain expected productivity levels while demonstrating teamwork and strong clinical judgment.
• Support the company's mission and vision while projecting a professional image.

• Bachelor’s or Master’s degree in Physical Therapy.
• Active Tennessee PT license.
• Current BLS/CPR certification.
