Join the team as a Physical Therapist (PT) PRN with Ahava Therapy Services.
Ahava Therapy Services offers Physical, Occupational, and Speech Therapy across a variety of care settings.
LocationDecatur, TN.
FacilityDecatur Wellness & Rehab – a 64‑bed skilled nursing facility.

- Review resident information and evaluate its impact on assessment and treatment.
- Conduct and document comprehensive functional assessments using standardized procedures.
- Develop and implement individualized treatment plans with achievable functional goals.
- Comply with evaluation, treatment, and documentation guidelines; maintain accurate records.
- Enter treatment and progress notes after each session.
- Revise treatment plans as needed based on resident progress.
- Document treatment course in clear, concise discharge summaries.
- Instruct assistants in treatment procedures and provide close supervision.
- Orient residents to program services and treatment procedures appropriate to their needs.
- Collaborate with nursing staff, residents, and families to maximize functional abilities.
- Instruct residents and caregivers on home exercises and therapeutic procedures.
- Assist in referral processes for additional services following discharge; maintain knowledge of community resources.
- Participate in rehabilitation team conferences, facility meetings, and quality improvement activities.
- Access continuing education opportunities to maintain clinical competency.
- Adhere to professional, regulatory, and facility standards.
- Maintain expected productivity levels while demonstrating teamwork and strong clinical judgment.
- Support the company's mission and vision while projecting a professional image.
- Bachelor’s or Master’s degree in Physical Therapy.
- Active Tennessee PT license.
- Current BLS/CPR certification.
