Marble City Medical is looking for an advanced practice clinician, APN or PA to join our team, caring for patients in skilled nursing, and long term care.
Conducting thorough physical examinations, reviewing medical histories, and assessing functional abilities to identify health concerns and develop individualized treatment plans for acute and chronic conditions in medically complex patients.
Diagnosing and prescribing appropriate treatment plans for acute and chronic medical conditions for medically complex patients
Providing routine care for chronic disease management for long term residents
Collaboration with nursing staff, therapists, social workers, specialists and attending physicians to coordinate optimal outcomes
Communicating effectively and promptly with all stakeholders, regarding health status, treatment plans, and other concerns.
Maintaining accurate, detailed and timely medical records, including progress notes, order entry, medical assessments, and treatment plans.
Requirements.
Nurse Practitioner/Physician Assistant/Geriatric Clinical Nurse Specialist
Active and unencumbered, TN medical licensing in good standing
Appropriate Board Certification, AANP/ANCC/NCCPA/ABSNC
Minimum 3 years experience in related field, with medically complex patients.
Unencumbered DEA license
Experience with electronic medical record systems
Able to work independently
Preferred applicant experience:
Inpatient (hospital and/or long term care) or ER experience is highly desirable
Specialized wound care, for elderly and medically complex patients
Independent contracting, 1099
Flexible work schedule
On-call schedule flexibility
