Community Primary Care Nurse

Support individuals, families, and communities as a Primary Care Nurse with Northern Health, providing direct clinical services across community settings. Work days, Monday through Thursday, 09:00 to 17:00.

This Registered Nurse role serves physician practices, homes, schools, and communities through Northern Health’s primary care services. The nurse applies best practices while working with an interprofessional team. Care follows each person’s primary Care Plan and personal goals, supporting collaboratively developed plans of care and ethical practice.

• Provide direct clinical care across community-based settings
• Serve individuals, groups, families, and communities
• Apply best practices to deliver competent patient care
• Conduct clinical follow up aligned with personal goals
• Inform collaborative plans of care with team members

• Registered Nurse designation
• Commitment to compassionate patient care
