We're in search of a caring Licensed Practical Nurse to join our dedicated health care team. This role involves coordinating Home Support Services while supervising Community Health Workers, advocating for clients’ needs, and implementing care plans. Your strong leadership and clinical knowledge will enhance team performance and client outcomes.

Key Responsibilities:

• Supervise Community Health Workers in service delivery

• Oversee implementation of individualized client care plans

• Provide training and evaluate personal care techniques

• Advocate for clients and facilitate health care access

• Collaborate with interprofessional teams for patient care

Requirements:

• Registered with BC College as practicing LPN

• Minimum two years related experience with supervisory role

• Current valid BC Driver’s License

• Knowledge in clinical specialties, such as palliative care

• Physical ability to perform nursing duties
