Position: Community Outreach Worker at Connective
Empower clients through relationship-building as a Community Outreach Worker with Connective. This role focuses on outreach support, planning, and client success for 20 hours weekly.

Connective is seeking a dedicated Outreach Worker to provide essential assistive support to community members. As a crucial member of the care team, you'll work directly with clients referred by our funders, helping them navigate daily living challenges. Your efforts will contribute to the creation of safe and inclusive communities, emphasizing a proactive approach to client well-being.

Key Responsibilities:

• Receive and manage client referrals effectively

• Collaborate to assess client needs and plan interventions

• Address everyday living issues in support planning

• Maintain clear records and documentation

• Participate in team meetings and ongoing collaboration

Requirements:

• Diploma or certificate in related field plus one year experience

• Ability to respond effectively to crisis situations

• Experience working with diverse client backgrounds

• Valid Class 5 driver’s license and reliable personal transportation

• Current Emergency First Aid Certificate required
