Outreach Worker

Position: Connective Outreach Worker Role
Connective is looking for an Outreach Worker to engage and support clients in their community journey. This part-time role offers 20 hours per week for dedicated individuals.

In this position, you'll assist individuals referred by Connective’s funders, ensuring they succeed in their transition and daily lives. You'll collaborate with the care team to develop comprehensive support plans, addressing critical areas like housing, nutrition, and mental health. Your contributions will directly impact building safe and inclusive environments for vulnerable populations.

Key Responsibilities:

• Assess client needs and formulate support strategies

• Address daily living issues in planning and intervention

• Maintain detailed records and monthly reports

• Liaise with community agencies for client services

• Participate in team meetings for service continuity

Requirements:

• Diploma or certificate in relevant field with related experience

• Proven ability to navigate crisis situations

• Knowledge of local community services

• Class 5 driver's license and personal insured vehicle required

• First Aid Certificate compliant with B.C. standards
