Work in the community setting at skilled nursing and assisted living facilities. Lead a team of Advanced Practice Providers and collaborate with an interdisciplinary team (Psychiatry, Psychotherapy, Podiatry, Wound Care where available).
Detailed Duties and Responsibilities- Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for family medicine, geriatric medicine, and/or internal medicine in long‑term care facilities.
- Review and sign MARs as requested by facilities; sign all orders specific for a supervising Physician while at the facility and or within 24 hours if faxed or emailed to include phone orders.
- Review and address labs within 24 hours of receipt; manage all scheduled medication prescriptions under Eventus Whole Health’s Scheduled Medication Rx Policy.
- Provide adequate time each visit for paperwork and administrative duties.
- Collaborate with facility to manage systems; attend care‑plan meetings and QA/PI (Quality Assurance/Performance Improvement) meetings as requested.
- Remain on call: available between 8 AM and 5 PM by text or phone on scheduled work days; triage policies and on‑call duties may be modified as needed.
- Submit copies of documentation and billing records to Eventus Whole Health according to billing schedules.
- Maintain open and positive communication with facility staff and administration; provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information.
- Attend meetings with administrators or designated facility care coordinators before and after patient visits.
- Adhere to Eventus Whole Health standards, policies, procedures, and all legal, regulatory, and ethical guidelines.
- Maintain patient confidentiality; comply with HIPAA rules, CPT coding guidelines, and appropriate use of cell phone, email, text messaging, patient charts, and EMR.
- Licensed MD or DO
- Minimum 3 years in long‑term care/geriatric care preferred
- AMDA certification a plus but not mandatory
- Strong leadership qualities
- Strong organizational skills
- Excellent written and verbal communication skills
