Position Summary

The CCBHC Crisis Diversion Specialist will provide case coordination, outreach, and advocacy services to individuals entering CCBHC. The role includes initial screening and referral services to connect consumers to needed resources. Clinics must provide comprehensive services for children and adults, including 24/7 crisis services; outpatient mental health and substance abuse treatment; immediate screenings, risk assessments, diagnoses; and care coordination with emergency rooms, law enforcement, and veteran groups.

The CCBHC model offers an integrated approach to reduce healthcare costs and improve patient outcomes. The specialist will provide direct and indirect care within the limits of training and credentialing under supervision.

Essential Duties/Responsibilities
  • Assist in conducting initial evaluations: assess consumer’s presenting problems and administer selected mental status exam items.
  • Participate as part of a 24‑hour crisis stabilization hub, including a mobile crisis unit.
  • Collaborate with law enforcement and community agencies to reduce the risk of emergency room and psychiatric hospitalization.
  • After stabilization, connect the individual to appropriate level of care.
  • Provide consumer education within area of expertise and credential limits.
  • Attend meetings and meet regularly with supervisor for supervision.
  • Maintain ethical conduct regarding confidentiality and professional relationships.
  • Perform clerical tasks: compose, edit, and type correspondence; enter and retrieve data; assist with reports and staffing sheets; negotiate billing/fees inquiries; retrieve clinical charts; prepare service slips; answer routine messages and calls.
  • Assist with answering inquiries, providing referral information, and routing calls appropriately.
  • Complete intake and registration; schedule appointments; instruct consumers/families on required documentation.
  • Verify consumer insurance and obtain prior authorization for inpatient stays; set up billing in EMR.
  • Maintain Crisis Diversion Daily Log.
  • Gather clinical information and coordinate with on‑call psychiatrist.
  • Demonstrate competent use of EMR.
  • Notify outpatient therapists of overnight and weekend contacts via emergency alerts.
  • Assist clinicians with screening emergencies and records.
  • Serve as a resource regarding available mental health services, referral sources, and emergency detention procedures.
  • Participate in cross‑training with other Crisis Diversion staff.
  • Perform other duties as assigned.
Minimum Qualifications /Requirements
  • One year experience in a crisis‑related field.
  • Strong oral and written communication skills.
  • Interpersonal skills.
  • Competence in assessment and treatment planning for medical and psychiatric patients in inpatient and outpatient settings.
  • Knowledge of the natural history of mental disorders and relevant biological and socio‑cultural factors.
  • Understanding of the range of treatment needed and appropriate utilization of available resources.
Certificates, Licenses, Registrations
  • A current American Red Cross CPR and First Aid certification.
  • A current Crisis Prevention Intervention (CPI) certification.
  • A valid driver’s license per HCI motor vehicle policy.
Physical Demands

The physical demands include managing emotional stress from working with distressed and unpredictable individuals, and making decisions regarding dangerousness, suicide potential, competency, and reality contact. Reasonable accommodations may be made for individuals with disabilities.

Work Environment
  • Overtime after 40 hours.
  • On‑call schedule supporting crises.
  • Night and weekend shifts periodically.
Conditions of Employment
  • Completion of tuberculin screening within four days prior to first day of employment and annually thereafter.
  • Completion of MMR, Varicella, influenza, and coronavirus vaccinations.
  • Pre‑employment drug screening; post‑employment tests upon reasonable suspicion.
  • Satisfactory reference and background checks.
  • Completion of Indiana Department of Child Services and Indiana State criminal background checks at hire and every four years for employees with direct contact with DCS clients.
