As a Hospice RN/ RN Case Manager, you will:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager and maintain primary control and professional management of each patient.
  • Act as the primary liaison between physician, patient/family, and hospice team.
  • Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload while building trusting and compassionate relationships.
  • Empower patient’s caregivers by educating them on care for their loved one.
  • Manage and collaborate with an interdisciplinary team including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to provide comfort, peace, and dignity.
  • Evaluate patient and family needs and assist in developing and implementing the plan of care for each patient.
  • Maintain all assigned patient clinical records accurately.
  • Participate in company quality assessment and performance improvement programs.
About You

Hospice Registered Nurse / RN Case Manager Requirements:

  • Must have a compassionate, kind, and empathetic heart for patients and their families.
  • Comfortable providing care wherever the patient calls “home”.
  • Demonstrate flexibility and composure in supporting others during crises.
  • RN license in the state of employment (or eligibility to obtain).
  • At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Additional experience in hospital settings such as Staff RN, Med Surg, PACU, ER, critical care, or telemetry is a plus.
  • Valid driver’s license and automobile insurance.
  • Current CPR certification.
